CERAMIC CLASS LIABILITY WAIVER AND RELEASE FORM

Studio Name: ONEHAPPYARTIST LLC

Location: 7001 E Main St, Ste 101, Scottsdale, Arizona 85251

1. Acknowledgment of Risks

I acknowledge that participating in ceramic classes involves working with tools, equipment, and materials (including but not limited to kilns, sharp tools, glazes, and clay) that can be hazardous. I understand the potential risks, including but not limited to: cuts, burns, slips, falls, allergic reactions, and inhalation of dust or chemicals.

2. Assumption of Risk

I voluntarily assume all risks associated with participating in these ceramic classes. I agree to follow all safety instructions provided by the instructor and understand that failure to do so may increase the risk of injury.

3. Release of Liability

In consideration for being permitted to participate in the ceramic classes, I hereby release and hold harmless ONEHAPPYARTIST LLC, its employees, contractors, instructors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may occur during or as a result of my participation.

4. Medical Treatment

In the event of an injury, I authorize ONEHAPPYARTIST LLC to seek medical treatment on my behalf if I am unable to do so and understand that I am responsible for any related expenses.

5. Property Damage

I understand that I am responsible for any damage I cause to the studio space, tools, or equipment through misuse or neglect.

6. Photography Release

I understand and agree that photos and videos may be taken during the ceramic classes for use in ONEHAPPYARTIST LLC’s promotional materials, website, and social media. By signing this waiver, I grant permission for my image and likeness to be used without compensation, and waive any rights to inspect or approve the finished product.

7. Agreement

I certify that I am at least 18 years old, and that I have read and understand this waiver. I agree to its terms knowingly and voluntarily.
